What is commonly called a blackhead is technically called an open comedone. This means that the opening to the suface is large and the contents is exposed to the surface. The comedone which you are asking about is actually called a "closed comedone" which means that it is not seen at the surface. This usually appears as a tiny, non painful bump under the skin.
